It’s funny, it’s just you don’t get it.
The child is in (what seems to be) a third world country and probably doesn’t get much to eat. Many people in first world countries smoke cannabis, or pot/weed, which gives them “the munchies” (hence the name of the picture). I hope you can understand it and laugh a little, even if it is at the expense of someone else.
